Financial Policy

When you schedule an appointment, you will receive an email confirming your appointment.

Next, you will receive an email with the link to a secure online questionnaire containing forms and consents to fill out prior to your appointment.

Please do not attempt to do pre-visit paperwork on the day of appointment, they do need to be completed for legal reasons before you are seen. Even if you are going to be seen in person, the forms are still electronic, so please fill them out in advance. If you just can’t make the online forms work, please let me know and we can use printed forms or send you PDFs instead. 

If you are new to the clinic, please go right ahead with filling out the forms, especially the intake form.  This form is long and detailed, but I will definitely review it before our appointment, and it will make the appointment much more productive. You don’t have to complete the form all in one sitting, you can save your progress and come back to it later. 

Financial Policy

  1. Dr. Krashin is considered an “out of network provider” for commercial and state-sponsored insurance plans other than L&I, and L&I appointments need to be booked through Labor and Industries directly.  
  2. Dr. Krashin is unable at this time to provide care for Medicare recipients.  
  3. The rates for services are listed separately on the booking page.  
  4. We ask all clients to provide credit or debit card information for their booked appointments.  The clinic uses the PaySimple online payment processor for all transactions. 
  5. We do not charge your card at time of booking. 
  6. Your card will be charged the full amount of the visit at the conclusion of that visit.  Your insurance carrier may provide partial reimbursement of services by out-of-network providers. This is more likely to happen with PPOs (Preferred Provider Organizations) than HMO plans.  Reimbursement rates vary and are controlled by the insurance plan. 
  7. If desired, patients will be given a “superbill” with all the necessary information to submit to the insurance carrier for reimbursement. 
  8. Insurance companies have differing policies and can change policies at any time; therefore, it is the patient’s responsibility to know the rules of their insurance carrier. It is a good idea to contact your insurance company ahead of time to find out about their coverage of out-of-network providers.
  9. Appointments may be rescheduled or cancelled online up to 24 hours in advance, see the scheduling policy for details.

Communications and Emergencies

There are three ways to contact the clinic: calling the main telephone, emailing the clinic, or sending messages via the secure portal. 

    • Important: if you are having an emergency, particularly a life-threatening issues such as a potential stroke or other concerning neurological symptom, or experiencing a mental health crisis, please call 911 or go the nearest Emergency Department for assessment and care.  The clinic cannot guarantee that messages, especially after hours messages will be responded to in a timely manner whether sent by email, phone, portal or other method. 
    • Email is not secure or HIPAA-compliant, and I strongly encourage using one of the other methods for any message containing sensitive personal or medical information. 
  • Email is not encrypted or secure, and is intended for general questions only, not therapy or medical advice. Correspondence by email does not constitute a doctor-patient relationship. Do not send urgent messages or cancelation requests by email. 
  • If you do not reach clinic staff by phone, you may leave a voicemail, please leave the patient’s first and last name, date of birth, and callback number. Messages will be checked every business day.  
  • Secure portal messages are the ideal method for communication as they are private and connected to your medical record. You can also book appointments through the portal. 

Rescheduling, Cancellation, and No-Show Policies

Note that when a patient appointment is booked, it is kept open for you. The clinic does not overbook appointments because everyone deserves the full attention of their doctor. This also means that if someone cancels at the last minute or doesn’t show up, the clinic is unable to fill that appointment time.  

  • You can reschedule or cancel your own appointments via the portal up to 24 hours of the appointment.
  • Within 24 hours, please notify the clinic by calling our number, sending a message via secure portal, or emailing the clinic.  
  • If you arrive more than halfway through a time slot for an appointment, it will also be considered a no-show (i.e. 15 minutes for a 30 minute visit)
  • The no-show/late cancellation fee is $50.  This fee is generally not reimbursable by insurance.
